I have the RSmail! Subscription plugin installed but cannot find any documentation on how to use it to create a subscription form.

First you should configure the fields for the subscription. To edit the fields, head over to Components > RSMail! > Lists and create/edit a list. More information about this can be found in the following Subscriber lists article.
Next step would be to create the RSMail! Newsletter module and publish it on your webpage. For this Head over to Administration panel > Extensions > Module manager > New module > create RSMail! Newsletter module.
In the module configuration screen, you will have the option to choose the list/lists and the fields which you want to be displayed in the module. More information can be found in the RSMail subscribe module article.
