I need include images to select into my forms, any simple solution?

Are you referring to associating images with radio/checkbox group selections? If this is the case, yes this can be done. You will need to configure the selection items as follows:

value1|caption1 <img src="path/to/your/image/file/here/1.jpg" alt="1"/>
value2|caption2 <img src="path/to/your/image/file/here/2.jpg" alt="2"/>
