I would like to have a separate registration form for each membership plan.

I.e.

Single membership to capture 1 individuals information. $25.00

Group membership with added fields to capture multiple individuals information. $100.00

Based on what membership you choose it will direct you to the appropriate registration form.

Is this possible?
