I figured out the front end of the form and it looks good. I can't figure out how to downlaod the information in a usable format. So far I have to customize excell every time I try to use the information.

Can anyone help me out.:unsure:
The administrator has disabled public write access.

If you have the Pro version you should be able to choose what fields to export.
