I can not get the form to show up when I click on the link it gives me this

This problem could be generated by the fact that you have an old version of MySql. If so, please update to 4.1.x or above.

I'm having the same problem with RSForm (not with RSTickets, which is also installed). Except mine is on line 129.
My MySQL version is 4.1.22-standard

EDIT - This is a good workaround: I realized a second form I had generated for inclusion in an article using {mosforme} worked just fine. So, I re-wrote the menu link to go to a new article containing nothing but my {mosforme 1} instruction. Works like a charm! I've got fatal error only when there are two forms on one page:

can this bug be fixed somehow?
