TOPIC: header and \"subform\"

Hello,

I have to set up a form (for a diving association) with a header :

with data like :
Name of the responsible
Location of the action
Date

It's what I call a header.


Then I need to fill a few times a \"subform\" like

Guide Name
member1
member2
member3
starting hour - ending hour
depth etc...

parameters of the dive

and I have to repeat this form up to 6 times but sometimes only one or two times.

I think I need a \"submit\" button and a \"new subform\" button and a \"end\" button
