maybe an idea to put up a form showcase? would love to see some forms in action! so thats the suggestion really...

Well,.... Don't know if \"the boss\" will like it.... but as long as you write down TEST in first or last name... then it's okay...

And if site isn't available on this like, then the site is LIVE, and then no more testing... hihihih.

nice form, layout is pretty neat too.

wouldnt it be nice to give them 2 options for picking the date,based upon the date whenever the events are? as in select the event they wanna attend and then give them the possible dates? dunno if its possible though

Yeah would be very nice, but have no clue how at this moment. So sorta have to leave it the way it is right now.

Still need to figure out how to change the color of the calendar. probably need to add something to my original CSS.

If you fill it in, you receive al the info you entered.
I put it on line, when i fixed \"Uw werkgever\" in a dynamic form.
If some one will choose anything else then NS, a new line must open with: \"Vul bankrekening nummer in\" Enter account number.

Please for testing naam and voornaam only with \"test\"
