How do you put 2 or more fields in the same \"row\", but diferent columns?

As an example, imagine you have 3 fields:

LASTNAME_FIELD FIRSTNAME_FIELD AGE_FIELD

I know i have to come up with a solution for that.. The actual form let's you create a common field template for all of the fields, but an individual template would be solution. I will think of a solution and let you know.
