I have 2 basic questions as I have just started using RS Form Pro

1. how do I turn off name of the module when I have displayed on my website (on website Contact Us_2025)
(I have turned it off in the module and it looks to be coming from initial setup from RS Form Pro but I cannot find where to turn it off)

2. Is there a way I can dress up/edit the "cpatcha layout"?
