TOPIC: Conditional Contact Form

I need to create a customer inquiry form that will route an email to the correct company department based on the type of issue selected from a drop down.

For instance, I'd like a field like this:
"My question is for: [radio button or drop-down] Sales/Service/Information"
And based on the field selected, have the form sent to the designated representative.

How would one go about doing such a thing?
