I would like to allow the form a Text Area where each line is an entry. And then for each line entered, a field for additional information would be asked.

Is this possible? If so, how?

The reason I am thinking Textarea, because the amount of entries is dynamic to the users situation. Maybe there is a better field type?
The administrator has disabled public write access.
