I need to set up a form where one question has 4 words. Users must choose which word most describes them and least describes them.

ie:
Question 1:
Most Least
brave ____ _____
inspiring ____ _____
submissive ____ _____
timid ____ _____

they would choose a radio button for which word most describes them, and a radio button for which word least describes them.

is this possible?
