TOPIC: Checkbox with images

I am trying to add images to a checkbox. The image width and height are not being used, as well as the caption being covered up by the image.

I am using:
value1|caption1 <img src="/images/stories/myimage1.jpg" width="69" height="84" alt="image1">
value2|caption2 <img src="/images/stories/myimage2.jpg"  width="69" height="84" alt="image">

In addition, I would like to be able to make this in 4 columns.

Any help is appreciated.
