TOPIC: Embedded Form Does Not Work

Hello:

I have an article with a form embedded in it here:
www.chesapeakefamily.com/index.php/fun/familyfavorites

Once you hit submit, the resulting URL is not working because the word "fun" is stuck in after the domain name. When I use the form in Preview mode, testing it works fine. This problem only happens when its embedded. Help! Thanks!
