I am creating a craft website for my niece and nephew similar to www.thewellrefinedhome.com/amys-craft-night.html. If I purchase RSForm!Pro what other plugin would I need to create these forms.
The administrator has disabled public write access.

You'll actually need some decent HTML and CSS knowledge to arrange your form like that. RSForm! Pro offers the ability to change the HTML code of your Form Layout (none of the pre-defined form layouts will work since your desired form has a different structure).
