Is it possible to include images on forms, these would just be jpegs or png's from a directory in the website directory.

Alsinterested to nknow if its possible to have a background image for a fomr.

And finally it would be interestng to know if it's possible to have a video in a form that can be watched before filling out the form?

Any links to examples would be great.
