TOPIC: Having images as dropdown selection

I need to make a form that allows for image selection in a dropdown. I will need to have a form like this:

Player Image | Player | Badges | Shaded Boxes | Points
Img 1 Img 2 Img 3 Img 4 Img 5
Img 6 Img 7 Img 8 Img 9 Img 10 Box 1 Box 2 Box 3

The Badges and Shaded Boxes fields will have images in a drop down to display badges earned and shaded boxes to display accomplishments. I need to make this for different teams and have all team players in each team listed on a single page.

How can I make the dropdown select an image?
