Hi,
When i send a form, always occur the same error message:
•The following from address failed: ......
•The following from address failed: ......
What's the matter?.

a couple possibilities:
1 - if you are working on your personal development server on your personal computer, localhost can't send emails
2 - if your form is setup to submit the form content to the database AND you have the form fields validating for unique entries, that might cause the error.

I've checked:

if your form is setup to submit the form to the database content and You Have the form fields validation for unique entries, That might cause the error.

And I turned off the fields with required input and the error message still appears.

As I can disable setup to submit the form to the database content?

I can do now?.

You're using SMTP (this can be checked in the Site > Global Configuration settings) and your SMTP server does not allow to use a different from address. You'll have to set "From" the same as the SMTP account (ie. you can't use placeholders to make the email appear as coming from the form submitter's email address).

We migrated our web site to a new server, and i'm having this exact same error as the OP. What's strange is I had this same email account configured exactly the same way on the other Joomla installation and I've never had this error, even when using a different From name.

That said, I went ahead and changed the From address to match the actual address and I still get the same error.
