I would like to show a group of fields when a value is entered in a single field.

For example, I'm developing a new client form for a veterinary clinic. They need to have owners be able to enter information for several pets. I'd like to have fields visible showing the "petName" field for up to 4 pets. If they enter a name in the first field, the rest of the fields required for that pet are made visible. I've experimented with doing this with conditional fields, but it's tedious to have to set it up for each individual field, and those can only be triggered by radio and checkbox fields - I want it to be triggered simply by entering a value in a textbox.

Any ideas how I might be able to accomplish this?

I have a form that I paid to have done a while ago that has a set of radio buttons that when each is selected a corresponding number of other form components appear. For example, if you select 2 you get 2 sets of areas to enter data.
If you'd like a .zip of this form to look at email me at This e-mail address is being protected from spambots. You need JavaScript enabled to view it and I'll send it to you.

Since I do not write code and paid to have this done, I won't be able to help you edit it to your needs. But someone who is familiar with coding should be able to do any edits to work for other requirements.
