TOPIC: Show additional field when Radio button selected

I found a few different ways of doing it, but now I'm trying to figure out how to integrate this into RSForm!Pro.

To give you an idea, it's for an RSVP form. I have a list of attendees which looks like:
John Smith *yes *no

I would like it that if the "yes" radio button is selected, an additional textbox loads under asking for an e-mail address.

Of course this field would have to be captured and show up in the Submissions.

Any thoughts/leads?
