Add shipping and additional tax options for memberships where a product is shipped to members

My client's membership plan includes shipping his product to monthly subscribers. He is required to charge state sales tax to customers in his home state; yet the current configuration only allows for tax across the board or no tax at all.

Also, having some shipping options would be great, too. As of now, we are simply building a flat rate of shipping into the membership price, which doesn't quite work out when they get international orders.
