Enhance conditional fields to include text input

In addition to "Dropdowns", "Radio Groups" or "Checkbox Groups" for conditional fields, add text input.
For example, here are 5 text inputs:
Staff Name #1
Staff Name #2
Staff Name #3
Staff Name #4
Staff Name #5
When Staff Name #1 is not blank, show fields Email #1, Emergency Contact #1 and Emergency Phone #1.
When Staff Name #2 is not blank, show fields Email #2, Emergency Contact#2 and Emergency Phone #2.
and so on.
