EDIT 2: After much testing, I eventually remembered, that RSForms is not compatible with Joomla cache. Almost the same as not compatible with Joomla ... U+1F644
What you're experiencing points to a caching issue. Please refer to this article that explains why a form might not work correctly with cache and what to do.
I have taken a look at your form and from what I can see, the Date Dropper field is displayed as the rest of your fields. Can you please properly refresh your browser cache (CTRL + F5) and test this one more time?

Hello! Write through the interpreter. I'm out of license for Your product, I like it. But I can not configure the cache on the Joomla site so that it does not interfere with the RSForm!Pro. Tell me whether that fixes the problem and if solved, tell me how can I achieve compatibility. I do not want to look for another product.
Try disabling or clearing your browser cache. That worked for me. Or try a different browser or incognito mode and you will see that it is working fine. That worked for me. Hope this helps.
You are using some CSS compression tools and it's possible your website CSS isn't being properly applied, thus the browser itself is the one what adds your numbers to your form layout since, in your case, it's composed out of ordered list elements.
Either try disabling such compression tools and clear your Joomla! cache to see if this helps, or try using some CSS on overwriting this:

I ve been trying all kinds of captchas, all of them are acriveted under plugins but non of them is shown in the frontend. In the backend>Components>RS Form it is shown.....
We encounter the problem as well since (I think) RSForm Pro 2.0.15. It looks like it's not in the system or content plugin, since their last update is months ago.
Hope this can be solved real quick, because otherwise we have to check all websites of our customers and disable the cache

2. I made changes in the form in joomla backend but in frontend changes are not displayed. I emptied browser and joomla cache but still no positive result.
I have a form in a module in place, right under a single article: this page
Now I want to grab the article title, and put it in the administrator e-mail.
I followed the instructions on this page and copied the code 'Getting the article's title' to: Manage Forms >> edit your form >> Properties >> PHP Scripts tab >> Script called on form process area
I have a hidden field in my form called: Subject
When I test the form and check the form submissions, the Subject-field does not...

Have you tried using the RS form pro module instead of the shortcode?
You could create a module and then load the module instead of the shortcode..
I have not tried it..
But since the menu item for contact form fixes the issue, maybe the module will also..

PS: Don't forget you can also disable the module cache (advanced tab), but it might not be needed.
